I am ______ and would like to greet you all. I am today presenting an interdisciplinary plan to address the major issues at Riverside Medical Center. This plan will enhance the coordination of care since it is the primary cause of confusion and ineffective communication between departments when changing shifts. As a charge nurse, I ensure that the patient needs are met, the assignment of nurses to different departments is controlled, and the communication between departments is effective. The idea is to assist the providers to enhance their collaboration, elevate patient outcomes, and retain nurses who enhance the functionality of the healthcare system (Paguio et al., 2020).
Slide 2
This communication plan is aimed at leveraging coordinated tools and measures to enhance the cooperation between the administration and nursing in Riverside Medical Centre when changing shifts. The most important goal is to make patient care easier through the exchange of information, manage workloads more effectively, and make wise decisions. The protocols are prepared by involving all the members of the organization to ensure that they are not left out. This strategy is intended to make sure that patient and medical staff needs are addressed and that all people will be transparent and accountable (Brenner et al., 2022). The charge nurse states that the absence of coordination between healthcare providers may lead to issues with transferring care in case of a change of shifts.

Organizational Issue
Riverside Medical Center has a high need to upgrade the communication departments due to the challenges that arise when shifts change, which slow down the delivery of care to patients and increase safety concerns. Providing significant updates on patients to administrative personnel is at times a challenge to nurses, and this can influence the quality of decisions and processes in the hospital. Nurses, administrators, IT specialists, and patient advocates will join forces to identify and adhere to appropriate guidelines of healthcare communications to resolve these issues. This new approach will result in improved treatment decisions and quality care delivery to the patients (Keskinocak & Savva, 2019). The charge nurse has noticed that it is hard to offer patient care on a continuous basis due to a lack of coordination between the healthcare providers.
Nursing and administrative employees should be able to effectively communicate with one another during shift changes in Riverside Medical Center so that they can have good treatment plans, continuity, and timeliness for patients. Once these improvements are made, the hospital can guarantee patient-centered care that will lead to improved outcomes and quality care of patients (None et al., 2024). The proposed procedures will enable the sharing of patient information across the departments without any complications.
Slide 4
Consequences of Not Addressing the Issue
Nurses are unable to report to administrative personnel regarding any critical issues related to patients that cause delays in care and treatment, and in making decisions. Disruption of communication negatively affects the functioning of the hospital, threatens the risk of medical errors, and the lives of patients (Yaacoub et al., 2020). Addressing these issues will lead to improved operations of the hospital and improved patient care.
The impact of poor interdepartmental communication in Riverside Medical Center has a negative impact on patients. The inability to attend to patient needs on time due to bad communication may lead to misunderstandings, the time required to respond to the situation will last longer, and in certain situations, the patient will be in danger (Pryce et al., 2021). When there is a breakdown in communication, the quality of care will be compromised and will result in issues in the delivery of efficient and on-time services, and possibly compromise the recovery of patients. By dealing with such issues, patient care will improve, and healthcare will be more efficient.

Relevance of Interdisciplinary Team Approach
The interdisciplinary team approach becomes very essential to facilitate communication between departments in the Riverside Medical Center with the shift change because it brings different outlooks together in an effort to solve the dilemma of lack of coordination in care. Its team ought to comprise nurses, IT experts, administrators, and patient advocates. Nurses give the actual experiences of taking care of the patients, administrators see the overall picture, and patient advocates maintain the view of what is in the best interest of the patients (Bhati et al., 2023). IT professionals play a pivotal role in ensuring that there is a smooth use of the necessary communication technologies to streamline the processes. Effective teamwork among such teams can enable them to attain improved patient outcomes without causing disorganized working processes.

Evidence-Based Interdisciplinary Plan Summary
Objective
The main goal of the interdisciplinary plan is to establish effective communication between departments in the process of the shift change at Riverside Medical Center. The key objective is to connect an electronic handoff tool with the electronic health record (EHR) system that will facilitate transferring and structuring the information about patients more conveniently across various departments. This is to prevent mistakes that can jeopardize the safety of the patients. The utilization of the EHR extension will be supported by having regular interdisciplinary conversations and practicing communication protocols on a daily basis (Engle et al., 2021). It focuses on holding healthcare processes more responsible and promoting effective teamwork, resulting in efficient and coordinated care that facilitates the efficient operation of the system and the patients.
Working
The communication improvement strategy involves a continuous improvement model that is organized and currently focused on the implementation of the electronic handoff tool. The team starts with the determination of the most crucial communication differences in the process of transition between the departments and establishes a particular objective to improve. By collecting data, the effect of the changes on collaboration and safety that are introduced after EHR changes is measured (Verma et al., 2021). Through such assessments, best practice is transformed and standardized, such as ensuring improved and successful updates and adequate handover documents.
Role of the Interdisciplinary Team
The Riverside Medical Center team consists of professionals representing different areas. The cross-disciplinary team of nurses, administrators, IT experts, and patient advocates applies their various experiences in the process of enhancing the coordination of care. Although administrators manage resources and organize what the organization needs to achieve, nurses identify and fix discrepancies in the manner in which people communicate who are involved in patient care. The patient advocates assist in involving the patients in the communication process, and IT specialists make it simpler to exchange information through the use of technology (Tamli & Sain, 2023). The collaborative work team assists in ensuring that effective communication methods are used by the departments as a means of accurate and timely exchanges. The hospital conducts frequent meetings concerning protocols and gathering the ideas of the employees, and the staff members can enhance the way they treat patients and how they handle their duties.
Slide 7
Implementation of Interdisciplinary Plan
By implementing the interdisciplinary strategy at Riverside Medical Center, a step-by-step approach will be used to ensure that the strategy is as successful as possible. It will be gathered into a group composed of nurses, administrators, IT specialists, and patient advocates, all having their tasks (Abbasinia et al., 2020). Regular meetings will be held to revise the communication plan to engage all departments and revise whenever needed in order to get it to work effectively in the day-to-day activities. IT experts will be brought in to train employees about the new communication tools and assist them in using them. When they collaborate, medical practitioners concentrate on enhancing the manner in which all people communicate and cooperate to deliver optimal outcomes to patients.
The effectiveness of the plan will be evaluated by assessing the satisfaction of the staff regularly, as well as patient safety, and the effectiveness of the communication, and finding areas to enhance. The team will also continue to give feedback to each other to improve their communication strategies and solve any problems. The work on this subject will include administrators who will lead the process, deal with resistance, and maintain the plan underway (Okolo et al., 2024). By applying the interdisciplinary approach, the team engages the concerned parties to participate in formalized actions that will enable them to communicate and collaborate effectively to achieve positive results in Riverside Medical Center.

Management of Human and Financial Resources
An interdisciplinary communication plan can only be effective when Riverside Medical Center can manage its people and finances effectively. The main aspects of the plan will be supported with the help of a proposed budget of $300,000. The budget will be used to recruit the right people to the tune of $30,000, various training programs will cost the organization 50,000, incentives like bonuses and development plans will be incurred to the tune of 120,000 and another 100,000 will be spent on technology, such as modern patient care systems and communication tools, to facilitate efficiency. The budget assists in the efficient utilization of resources to ensure that the change plan of strengthening collaboration, simplification of work, and better patient care outcomes can be successfully implemented (Stefan et al., 2024).
Similarly, Riverside Medical Center has implemented the same programs to assist with communication and improve the organization of the change between shifts. High-level communication technology has been added, including HER systems and secure messaging, to enhance the collaboration of various healthcare teams. The hospital has also invested in its staff development and training, which has assisted in reducing staff turnover and improving their job performance (Singh et al., 2024). The results of improved collaboration, improved decisions, and improved patient care are evident, hence the need for technology and staff investment to adopt an efficient healthcare environment.
Slide 9
Criteria to Evaluate the Project Success
In the Riverside Medical Center, several evidence-based measures will be used in the assessment of the success of the communication improvement plan. Nursing and administrative staff will receive surveys at the beginning to determine whether the new modes of communication are as expected by them. Through this, the two departments will be aware that they are supported and that they have everything necessary to provide excellent patient care. In addition, patient safety provisions, such as preventing drug errors, quickening responses, and delivering prompt services to severely ill patients, will be monitored in order to evaluate how the enhancement of communication leads to enhanced patient outcomes (Mostafa & El-Atawi, 2024). Through these evaluations, professionals will be able to guide additional corrections to the communication process, which will keep the care outcomes high.
The contribution of the communication improvement plan will also be researched in Riverside Medical Center by observing the efficiency enhancement. It is knowledge of the amount of time, and the speed of the staff in caring about the patients, whether the shift change and the improved collaboration of different departments are smoother. In addition, the hospital will monitor medication adherence of the patients, as well as the reduction of readmissions, to determine whether the changes are enhancing patient outcomes (Cram et al., 2022). The set standards will confirm the beneficial effect of the communication plan on patient care, on employee satisfaction, and on the management of the hospital.
