Assessment Plan Overview

Assessment 1: Finalise project template and initial project proposal. Assessment 2: Develop a clear PICOT question and rationale. Assessment 3: Identify 1 intervention (nurse-led lifestyle programme). Assessment 4: Talk about the strategy for implementation. Assessment 5: Create an evaluation plan to guide success. Assessment 6: Put together papers and create a dissemination plan.

Practicum Hour Expectations

Cumulative – (Assessment 2 40 hours, Assessment 3 80 hours, etc) 1 sentence written for every 2 hours logged. Must include specific activity (e.g., going to meetings, talking about patient care plans).

Documentation Reminders

For all assessments starting with Assessment 2: Provide an Excel log of hours. Be sure the preceptor confirms hours. Do not use general terms, such as ‘shadowed preceptor.

Preceptor and Site Engagement

Most of the Practicum will happen on-site with the preceptor’s guidance and supervision, with up to 20% of hours remote (e.g., via Teams or Zoom). Identify leadership and/or clinical improvement meetings to attend.
