Introduction

The capstone project is related with creating a chronic hypertension patient education program to indicate the problem of hypertension in the community. Global statistics reveal that hypertension or high blood pressure is a prevalent condition that increases the risk of developing various cardiovascular diseases, stroke, and an array of comorbidities (Fuchs & Whelton, 2019). The rationale for this project is the necessity of raising the efficiency of patient care and incorporating effective educational management concepts and techniques. By catering for a structured patient education program, patients can be equipped with adequate knowledge and skills that will help them carry out their wellbeing routines hence reducing the possibilities of the complications that may arise due to the disease. 

I need to acknowledge that performing the background analysis has its importance for several reasons. Firstly, it offers an insight into real-life practice, missed opportunities in points of care, and areas in hypertension care and patient education that may be in the developmental stages. Second, the concise systematic background check gives an assurance of professional, professional specialty, and sub-specialty benchmarks to embrace the evidence and scholarly content in the delivery of practical healthcare services (Fuchs & Whelton, 2019). In conclusion, the capstone project background analysis contributes to the development of the patient education program by advancing knowledge regarding the patient population and their needs.

Synthesis of Background Analysis

Background analysis of the patient education program for the patient diagnosed with chronic hypertension must include the fundamental points concerning hypertension management and education within the framework of nursing. Prolonged hypertension, whereby patients continuously experience high blood pressure, poses a threat to health, particularly because it contributes to the development of cardiovascular diseases and other related conditions (Madhur et al., 2021). It also demonstrates how managing hypertension became not only about patients’ clinical parameters including blood pressure and medication management but also focused on aspects of patient education aimed at enhancing self-management and lifestyle management. 

This background analysis provides a systematic assessment of current procedures and the existing best practices and guidelines in hypertension management. This comprises an examination of blood pressure, principles of hypertension, and the pharmacological and nonpharmacological management options (Fu et al., 2020). For example, patient education includes informing the patient on matters such as why he/she has to take his/her medicine, why he/she has to change the kind of food he/she eats, how he/she has to exercise, and how to manage stress to ensure his/her blood pressure levels remain within an acceptable range and, thus, no complications. 

Furthermore, incorporating patient-centered care concepts is key in managing patients with hypertension as they require personalized attention. This means encouraging patient-centered care and engaging patients in planning for their future care, as well as making decisions around treatment and adjustments to their behavior (Scherer et al., 2021). The synthesis also underlines that there is miraculously integrated cooperation between the healthcare professionals to conform to the patient-centered approach, which is ultimately crucial for the recovery of the patient.

Impact on Personal Practice

One of the aspects of the capstone project, which I propose to create a chronic hypertension patient education program, is likely to have a strong influence on my, as a nurse, practice in the following points: First, it will improve my communication with patients regarding their condition, especially when it comes to simplifying the information about hypertension control for patients to understand (Koopman et al., 2020). They pointed out that rapport is critical to developing trust with patients while also explicating the reasons why patients need to follow instructions by health care providers and other advisable lifestyle changes that may be necessary. With the enhanced interpersonal communication skills that I will learn in this project, I will ensure that patients are equipped with adequate information and knowledge on how to handle hypertensive illness thus resulting in better health. Secondly, the opportunity to employ educational techniques that were discussed in the course of the project will enhance the general competency of patient education delivery (Kristjansdottir et al., 2021). This requires incorporating evidence-based research and guidelines to develop other educational materials and sessions that are useful and appropriate for hypertensive patients. Therefore, the principles of evidence-based practices, like the focus on specific aspects of medication management and lifestyle modifications supported by the research, would allow me to guarantee that the education provided has not only informational value but also positive behavioral modification outcomes in patients. Finally, the success of the project will benefit the institution by enhancing my skills in the management of chronic diseases. Since hypertension is a chronic disease that patients may have for their entire life, it is effective to build competence in managing this condition since it will be more beneficial to me as a nurse. Along with this, we should also be able to educate on high blood pressure status, signs of side effects from medications, and how to cope with daily struggles with compliance. Through the capstone project, I will be able to improve the targeted areas to express a higher level of responsive, well-rounded care for patients with CH by meeting the existing and future needs of these clients.

Impact on the Nursing Profession

The creation of a chronic hypertension patient through the capstone project and education program has far-reaching impacts on the progression of the nursing profession in the following main aspects. Firstly, it claims an appropriately large stake in the accumulation of Evidence Practice (EP) within the sphere of nursing (Ihlebæk, 2022). Thus to create a strong background of EBP in hypertension treatment, nurses should take their time to research and implement the current best practices in the teaching approaches towards the patients. This not only improves patients’ outcomes by providing care from competent personnel but also refutes the argument stating that healthcare organizations are rigid and unchanging.

Secondly, the enhancement of patient education can also be considered as one of the project’s goals, as this activity may contribute to the setting of new standards in this field. Patient education can be regarded as one of the critical elements that can enable a patient to control their conditions in case of chronic diseases such as hypertension. Given this, nurses, especially those who are directly interacting with clients, are tasked with the creation of educational initiatives that engage clients while, at the same time, raising awareness that is based on the needs of clients (Hämel et al., 2022). With this undertaking, nurses are liberated to come up with creative ways of delivering health education to the patients; which can automatically translate to better patients’ health status and satisfaction.

The capstone project can help with potential decreases in the cost providers incur concerning hypertension treatment. By increasing the patients’ awareness of their conditions and compliance with the prescribed treatment regimens, the nurses will be in a better position to reduce some of the associated complications and hospitalization caused by uncontrolled hypertension (Paczkowska et al., 2021). This preventive approach is beneficial to the patient’s well-being as well as to the current overburdened healthcare system; making it clear how proactive the profession of nursing is in reshaping current healthcare models. In general, the given project’s outcomes affect not only the specific patient’s individualized treatments and courses in nursing but also cause overall evolutions in nursing as a discipline and as a segment of healthcare systems to produce increased advancements for the greater benefit of patients and other aspects of the nursing domains.

In conclusion, it is crucial to provide a background to the capstone project on chronic hypertension patient education as it will form the basis for making significant decisions and sufficient planning. Analyzing the features and existing deficiencies in hypertension treatment, nurses would be able to create specific measures that are suitable for the individual patient’s needs. Such a process improves not only the quality of patient care but also aims at the development of the thinking and research skills of nursing professionals. Personally, it provides better awareness of evidence-based practice and patient-oriented approaches as well as apprising the nurses with many instruments to face different kinds of issues in the field of healthcare. The clinical application of the project can be summarized as the following benefits: Professionally, there is an improvement in its standard of nursing practice from patient education to cost-savvy applications. In conclusion, it is crucial to conduct a strong background analysis to make sure the capstone project is not only significant but also has the potential to positively affect patients’ outcomes and different healthcare systems.
