Introduction
Nursing theories give a framework for directing the practice, training, and research in the field of nursing. They provide a structured approach of analyzing and describing the nursing phenomena and ease identification of the specific nature of the nursing profession(Fawcett, 2021). This response will compare and contrast grand nursing theories with grand, middle, and micro theories; discuss the nurse’s role in one specific theoretical framework; evaluate one theory using a specific framework; and explain the importance of learning about nursing theory.
Differences Between Grand Nursing Theories and Other Theories
Nursing theories can be classified into three main categories based on their scope and level of abstraction: They are grand theories, middle range theories or conceptual models and practice-level theories(Fawcett, 2021). The key differences between grand nursing theories and other types of theories are:
Scope: Grand theories cover the full range of the nursing metaparadigm concepts of person, nursing, health and the environment while the middle range concept focuses are narrower and deal with certain phenomena(Wayne, 2024). Finally, the practice-level theories are the most restricted in terms of specialization or area of interest; they are designed for specific nursing scenarios or clients.
Abstraction: Grand theories being more complex, and broader in nature and provide a holistic view of nursing. Because as compared to grand theories, the middle-range theories are not so broad and general they are easier to test because they are framed based on few concepts and propositions(Wayne, 2024). The final category of theories is less abstract and more concrete that mostly pertains to the nursing domain and concerns, and provides clear guidelines for practice.
Testability: Grand theories are also very hard to test empirically since they are very general and highly abstract in nature. It proposes a general framework of knowledge about nursing; however, the hypotheses that can be obtained from this theory are rather difficult to prove(Wayne, 2024). As compared to grand theories, middle range theories are easier to research because they contain less variables(Wayne, 2024). The most accessible type of a theory is practice-level theories, as these describe nursing practice and are applied and assessed in terms of practical situations.
Purpose: Grand theories are also useful in giving a broader view of nursing and its framework for constructing the body of nursing knowledge. Middle-range theories are more specific than grand theories and help to direct both research and practice in particular domains(Fawcett, 2021). practice-level theories are meant to assist nursing practice dealing with particular settings and people.
Role of the Nurse in Nursing Theories
The scope of practice for the nurse depends with the type of theoretical frameworks in use. For instance, in the theory developed by Lydia Hall, it is highlighted that a nurse is mainly involved in what is referred to as the “care” constancy which consists in offering close compassionate care and emotional support, offering pt education, and helping them with the basic needs at their home(Ahtisham & Shannon, 2019). Thus, the role of the nurse is different to the “cure” of the circle which entails medical cure and undertakings. In this theory, the nurse plays a role of an enabler for patients so that they may be able to care for themselves again.
According to another grand theory, Neuman’s Systems Model, the role of a nurse consists of evaluating stressors and implementing primary, secondary, or tertiary prevention to achieve, advertise, or regain the optimum stability of the client system. The nurse also engages the patient and other health care providers to assess stressors within the patient’s environment which may have an effect of the patient’s health status(Montano, 2020). Nurse also has an advocacy role where they work to fulfil patients’ needs and engage the patient in all decision-making regarding the care they receive.
According to the Watson’s Theory of Human Caring, the genuine role of a nurse is to establish a caring presence that fosters the process of healing. The nurse enters into transpersonal caring relationships with patients though adopting caring processes such as listening, understanding and valuing the patient(Bagheri et al., 2023). The nurse also helps patients to search for meaning in what has happened to them and assists with the spiritual aspect of their care.
Theory Analysis Using Smith’s Framework
From Smith’s textbook (Chapter 3) theoretical framework that was suggested for the analysis of theories, one can analyze a theory like Neuman’s Systems Model (Smith, 2019). This grand theory posit the patient as an open system receiving stressors from the internal and external environment. The nurse is responsible for evaluating stressors and for implementing primary, secondary, or tertiary prevention to achieve, maintain, or regain a healthy stability of the client system(Eustace, 2021). The ideas embraced by the theory could be compared and contrasted for their sensibility, precision, and coherence. It might also be possible to assess its testability, empirical indicators, and pragmatic adequacy.
When analyzing Neuman’s Systems Model using Smith’s framework, one could consider the following:
Clarity: The ideas of the client system, stressors, and prevention are well explained and consistently applied in the framework of the presented theory(Eustace, 2021). However, there will always be certain ideas that can be rather resultant in nature; for example, an optimum client system stability.
Consistency: The propositions of the theory are logically coherent with the assumption and concepts of the theory(Eustace, 2021). For instance, the idea of the proposition that states that nurses should use primary, secondary as well as tertiary prevention in order for the client system to attain, maintain or re-attain its optimum stability is also in harmony with the theory that regards the patient as an open client system that has interactions with stressors.
Testability: Although the theory gives a holistic view of nursing practice, some of the propositions suggested in it may be challenging to assess in practice because they are broad and complex (Smith, 2019). But general hypotheses deduced from the theory could be empirically tested or posited by the theory.
Empirical indicators: The theory is somewhat helpful in offering suggestions on how to operationalize its central concepts for empirical study (Smith, 2019). For example, quantifying stressors and clients’ system stability.
Pragmatic adequacy: The theory has been used in practice and education and referenced in published research, which implies that the theory is practically adequate (Smith, 2019). Although the framework contains a vast array of strategies, it may not easily be applied to particular nursing scenarios.
Personal Reflection
Analyzing nursing theories through the framework developed in Smith’s textbook facilitates a critical appreciation of specific aspects of the theory, which may include assumptions, concepts and propositions. Another advantage is that the reader can assess the clarity, coherence, and relevance of the theory to nursing practice. When comparing a range of theories from the grand to the middle-range and putting them into practice, this framework identifies the specifics of all the theories and their advantages for constructing nursing knowledge and enhancing the practice.
This theory analysis as a nursing student has given me a perspective of the various theories, and how they can be useful in practice. The explanation of the distinctions between grand, middle-range, and practice theories allows one to consider and use the theories suitable for the practice context and patient groups. For instance, in my clinical rotations, the Theory of Human Caring developed by Watson is helpful in shaping how I engage with patients and facilitate a caring atmosphere that fosters patient growth.
Theory analysis has also assisted me to cultivate critical evaluation skills whereby I assess the effectiveness of theories and their relevance in practice. Through this process, I was able to develop the understanding of the use of theory in nursing and its contribution to the development of evidence-based practice.
Conclusion
Many scholars argue that nursing theories are important in the growth of the profession of nursing. This paper has aimed to compare and contrast grand, middle-range and practice theories and understand their characteristics so that nurses can choose which theory to adopt depending on the context of the practice setting or the population being served. Performing theory analysis based on a structured frame allows for critical thinking and improves the ability of a nurse to apply theory and research to practice for achieving desired patient outcomes.
Therefore, the Nursing theories will be useful in the growth of this noble profession, especially in determining and articulating the role of the nurses in this health care facilities and in enhancing the quality of the patients’ care that is being offered by nurses. Nursing theory and the analysis of nursing theory empowers nurses to assist in the development of the profession as well as enhance the lives of patient populations.
